Support Coordinator

Warragul VIC | Hybrid 15 Month Fixed-Term Contract | Maternity Leave Cover We are looking for an experienced Support Coordinator to join our team in Warragul on a 15-month fixed-term contract, providing maternity leave cover.

This is a hybrid role, offering a mix of office-based and work-from-home arrangements while supporting participants across the Gippsland region.

About the role

As a Support Coordinator, you will work closely with NDIS participants to help them understand and implement their plans, connect with suitable services and supports, and build their confidence to navigate the NDIS.

Your responsibilities will include
Managing a caseload of NDIS participants
Supporting participants to understand and implement their NDIS plans
Connecting participants with suitable providers and community supports
Building solid relationships with participants, families, providers and other stakeholders
Monitoring plan utilisation and participant outcomes
Assisting with plan reviews and gathering supporting information
Maintaining accurate case notes, records and documentation
Working collaboratively with internal and external stakeholders

About you

We are looking for someone who:




Has experience in Support Coordination or a similar NDIS role
Has a good understanding of the NDIS and participant plans
Is confident managing their own caseload and priorities
Has solid communication and relationship-building skills
Is organised and comfortable working independently
Has good written documentation and computer skills
Takes a practical and participant-focused approach to their work
