Join us as a Community Aged Care Assessment Partner and be part of something bigger.Who are we looking for?
We're looking for a confident and experienced clinician with a minimum of two years working autonomously in the field to take on this role that will work with clients in the community while staying connected to a supportive, multidisciplinary team.You will enjoy getting results for your clients as a result of your ability to conduct thorough assessments and applying sound clinical reasoning skills, which in turn will help older people maintain independence and quality of life at home.You're someone who:Builds trust quickly with customers, families and stakeholdersIs confident working independently and making decisions in complex or changing situationsHas proven experience managing a wide-ranging case load of clients along with the associated workloadCommunicates with clarity, empathy and professionalismWhat can you expect to be doing?
Reporting to the Care Community Lead, this role will see you:Conducting in-home assessments and reviews to guide care planningCoordinating your own calendar and managing daily visits across the communitySupporting person-centered care focused on independence and re-enablementProviding sound clinical advice and guidance on complex or escalated care needsCollaborating with internal teams, providers and health professionals when requiredWhat do you need to bring?
To thrive in this role,
you will have:Tertiary qualification in Nursing, Allied Health or related fieldCurrent AHPRA registration (where applicable)Experience in community aged care or complex care settingsStrong assessment, care planning and clinical decision-making skillsAbility to work autonomously and manage competing prioritiesWe would also love you to have:Experience working within a wellness and re-enablement modelConfidence in case conferencing and multidisciplinary collaborationStrong documentation and reporting skillsWho is AnglicareSA?
