Job DescriptionWe are seeking a compassionate and experienced Registered Nurse to join our Homecare team, supporting the delivery of high-quality clinical care to clients living independently in the community.Reporting to the Customer Care Manager, you will play a key role in providing clinical coordination, case management, and nursing support to Homecare Package clients.
This role is ideal for someone who enjoys a mix of clinical work, care planning, and stakeholder engagement.Key ResponsibilitiesDeliver high-quality, person-centred nursing care to clients in their homesConduct assessments, develop and review care plans to meet changing client needsProvide clinical guidance and respond to changes in client health statusMaintain accurate and compliant clinical documentationCollaborate with multidisciplinary teams to ensure optimal care outcomesBuild strong relationships with clients, carers, GPs, and community stakeholdersSupport continuous improvement initiatives and uphold best practice standardsEnsure compliance with Aged Care Quality Standards, WHS,



and organisational policiesParticipate in mandatory training and ongoing skilled developmentAbout YouYou are a dedicated and proactive Registered Nurse who is passionate about supporting older people and individuals living with disability to live independently.Strong clinical knowledge and sound judgementExcellent communication and relationship-building skillsThe ability to work autonomously and within a teamA genuine commitment to quality care and continuous improvementKey RequirementsBachelor of Nursing with current, unrestricted AHPRA registrationMinimum 5 years' post-graduate experience (aged care, community, or hospital setting)Experience in wound care, catheter care, assessments, and care planningSolid understanding of aged care standards, WHS, and risk managementCurrent First Aid Certificate and National Police CheckValid driver's licence and access to a reliable vehicleWhy Join HCA?
