We're proud to be building a workplace where individuals feel empowered, supported, and valued.Role OverviewWe're seeking a passionate and experienced Clinician – Restorative Care to hold and manage a full restorative clinical caseload within our Support at Home Restorative Care Pathway.Reporting to Head of Clinical Governance you will lead each restorative episode end-to-end from comprehensive assessment through to structured completion within defined timeframes (up to 16 weeks).
You will deliver intensive, goal-directed care focused on functional improvement, reablement, and independence.Grounded in restorative philosophy, this role champions approaches that build or restore participant capacity – focusing on what people can do and supporting them to do more.
You will provide clinical governance oversight across allocated episodes, ensure funding is utilised purposefully and in alignment with agreed restorative goals, and strengthen pathway integrity across the broader Care function.This is an exciting opportunity for a Registered Nurse or Allied Health qualified who thrives in clinical leadership, values measurable outcomes, and is motivated to drive model fidelity, compliance, and continuous improvement in a growing and innovative aged care organisation.What You'll Do
Hold and manage a full restorative clinical caseload, leading each episode from assessment to structured completion
Conduct comprehensive clinical assessments and establish functional baselines
Develop structured, time-limited restorative goal plans aligned to participant outcomes
Deliver intensive, purposeful, outcome-focused episodes grounded in reablement principles
Lead multidisciplinary coordination and case conferencing to ensure aligned, timely service delivery
Partner with participants, families,
allied health providers, and third-party services to deliver person-centred care
Initiate exit planning at commencement of each episode and document clear recommendations at completion
Provide ongoing clinical monitoring, formal goal evaluation, and plan adjustments in response to progress or risk
Provide clinical governance oversight across allocated restorative episodes, ensuring quality, safety, and model fidelity
Oversee documentation standards, care management activities, and funding utilisation in line with pathway and legislative requirements
Escalate clinical risks in accordance with governance protocols and support incident management processes
Contribute to audit readiness, regulatory compliance, and continuous pathway improvement initiatives
Design and deliver education to Care team members to strengthen restorative identification, referral quality, and pathway activation
Promote dignity of risk, supported decision-making, cultural safety, and restorative philosophy across the Care function
Ensure alignment with the Aged Care Act ****, Support at Home Program Manual, and Aged Care Quality Standards
What You'll Bring
Registered Nurse or relevant Allied Health professional with current AHPRA registration
Minimum 4 years' post-graduate clinical experience
Demonstrated experience managing complex, multidisciplinary cases
Case management experience in rehabilitation, chronic disease management, or episode-based care
Strong clinical governance knowledge and risk management capability
Experience overseeing documentation quality and regulatory compliance
Proven ability to work collaboratively across care, quality, and operational teams
Strong understanding of funding stewardship and outcome accountability
A restorative and reablement mindset, focused on measurable participant outcomes
High level of qualified integrity and commitment to person-centred care
Desirable:
Experience within Support at Home or Home Care programs
Experience supervising clinical care or multidisciplinary teams
Postgraduate qualifications in gerontology, rehabilitation, or care coordination
Experience working within geographically dispersed teams
