About the role

Total Health Support is seeking reliable and motivated Disability Support Workers and Day Options Facilitators to join our growing team in Adelaide. Supporting NDIS participants with community access, daily living activities, personal care where required, transport, meal preparation, skill development, social participation and implementation of individual support and behaviour support plans.

Day Options

Facilitators will also plan and deliver meaningful individual and group activities such as cooking, life skills, community outings, art and craft, recreation, social activities and programs that promote independence and participant goals.

Key responsibilities
Support NDIS participants with community access
Assist with daily living activities
Provide personal care where required
Arrange and provide transport
Prepare meals




Support skill development
Facilitate social participation
Implement individual support and behaviour support plans
Plan and deliver meaningful individual and group activities
Deliver programs that promote independence and participant goals

About you
Caring, reliable and skilled
Positive communication skills
Comfortable working independently and as part of a team
NDIS Worker Screening Check
Working With Children Check where required
Current First Aid and CPR
Australian driver's licence
Reliable vehicle
NDIS Worker Orientation Modules
Relevant disability/community services experience preferred
