Job DescriptionAbout the roleRoss Care is seeking a dedicated and compassionate Service Coordinator to join our team in Prospect SA on a full-time basis.
In this pivotal role, you will be instrumental in coordinating and delivering high-quality disability support services to our clients.
As a Service Coordinator, you will act as a key liaison between clients, staff, families, and external service providers, ensuring that individualised care plans are effectively implemented and that every client receives the support they need to maintain independence, dignity, and quality of life.
This position is essential to our operations, directly impacting the wellbeing and satisfaction of vulnerable members of our community.Key responsibilitiesCoordinate client appointments, home visits, and support services in accordance with individualised care plansConduct initial client assessments to identify support needs and develop appropriate, tailored support plansMaintain accurate and up-to-date client records and documentation in compliance with relevant legislation and organisational policiesCommunicate effectively with clients, families, carers, and support staff to ensure explicit understanding of services and expectationsMonitor client progress and adjust service delivery as required to ensure optimal outcomes and client satisfactionLiaise with external service providers, healthcare professionals, and community agencies to coordinate integrated careHandle client inquiries and concerns with empathy and professionalism, resolving issues promptly or escalating appropriatelyMaintain compliance with disability support regulations, standards, and safeguarding requirementsAssist in the training of support workers as neededPrepare reports and data for management review and quality assurance processesWhat we're looking forRelevant qualifications or certification in aged care, disability support, community services,



or a related field (or demonstrated equivalent experience)Proven experience in a coordinating or supervisory role within aged care, disability support, or community services sectorsStrong organisational and time management skills with the ability to manage multiple clients and priorities simultaneouslyExcellent written and verbal communication skills to liaise effectively with diverse stakeholdersDemonstrated understanding of disability support principles, including person-centred care and dignity of riskSound knowledge of relevant legislation and standards, including the Disability Discrimination Act, and privacy legislationProficiency with computer systems and database management softwareA compassionate and empathetic approach to working with vulnerable populationsCurrent Working with Children Check, NDIS Worker Screening Check (or willingness to obtain)Excellent problem-solving abilities and sound judgment in complex situationsAbout usRoss Care is a respected community services organisation dedicated to delivering compassionate, high-quality support to people with disabilities.
