Baptist Care NSW & ACT is seeking a dedicated Tailored Support Coordinator for a permanent part time role (4 days per week). You will support tenants and households in accessing services to achieve their self-identified goals and sustain tenancies.

You will work with tenants, community services and stakeholders to design tailored support plans, provide warm referrals, advocacy and monitoring. This role emphasizes trauma-informed, person-centered care and cooperative teamwork.
