We are seeking a proactive and engaged Residential Care Case Manager to join our Residential Services team.

About the Role
As the
Residential Care Case Manager
, you will:
Provide case management support to young people in residential care and their families to achieve positive outcomes and greater independence.
Coordinate and monitor individual care plans, working collaboratively with residential care staff, DFFH and external stakeholders to deliver integrated support.
Oversee the Lead Tenant Program, including the recruitment,
supervision and support of Lead Tenants and the case management of participating young people.
Advocate for young people and facilitate access to services, resources and opportunities that support their wellbeing, development and transition to adulthood.

This role can involve emotionally demanding situations and exposure to psychological hazards, including client distress, trauma disclosures, and urgent responses.
MASP prioritises staff wellbeing by providing supervision, debriefing, reflective practice, safety measures, and access to wellbeing resources.
We encourage open communication—raise any concerns with your supervisor, management, or the OHS Committee as soon as possible.
Remuneration
Expected commencing salary of
$95,****** per annum
plus statutory superannuation.

Skills And Experiences
About You
To be successful in this role, you will have:
Relevant tertiary qualifications in Social Work, Community Services, Social Science, or other relevant tertiary studies.
Demonstrated experience in case management with young people, families and complex support needs.
Strong knowledge of trauma-informed practice, child protection frameworks and Best Interests case planning.
Understanding of the needs of adolescents and young people with experiences of trauma, out-of-home care and child protection involvement.
Strong communication and interpersonal skills
