Monash Health is seeking an experienced Nurse Practitioner to join the Community team on a fixed-term, 12-month parental-leave coverage. You will provide advanced and autonomous clinical care across Emergency Department and community programs to prevent avoidable presentations and support protected discharge at home.

The role combines ED practice with clinic-based care and outreach, including home visits within the Monash Health catchment.
