Eyre and Far North Local Health Network (EFNLHN) seeks a compassionate Case Coordinator or Care Partner - Support at Home for Streaky Bay and Ceduna. The role supports CHSP and Support at Home programs, coordinating care plans and coordinating with multidisciplinary teams to promote safety and wellbeing.

The position is full time, based in a rural area, with an EN stream under RN supervision or an ASO3 stream, depending on qualifications and experience.
