Northern NSW Local Health District is seeking an experienced Nurse Practitioner for a temporary part-time role (93 weeks) to lead an creative hospital avoidance model and deliver patient-centered care in a hub-and-spoke framework. With 22 hours weekly, you will work with local referrers and community services to improve access and outcomes.
