What you'll be doing
The Dietician provides advanced clinical leadership and high-quality, evidence-based, person-centred nutrition services within the Transition Aged Care Program (TACP) and Aged Care Transitional Intervention Program (ACTIP) in the Hastings Macleay Clinical Network.
The role supports older people with complex health and care needs to optimise nutritional status, independence, and health outcomes following hospital discharge.
About You
You are an experienced and compassionate Dietitian with recognised qualifications in Nutrition and Dietetics and eligibility for full membership with Dietitians Australia.
With extensive clinical experience supporting older adults, you have developed a strong understanding of age-related nutritional challenges and the application of contemporary dietetic practices to improve health outcomes, promote independence, and enhance quality of life.
You are passionate about delivering person-centred, evidence-based nutrition care and are confident managing complex clinical situations both independently and as part of a multidisciplinary team.
Your ability to assess nutritional needs, develop individualised care plans, and provide practical interventions enables you to make a meaningful difference to the wellbeing of older people across a variety of healthcare settings.
Recognised for your excellent communication, education, and relationship-building skills, you are adept at engaging with patients, families, carers,



and healthcare professionals.
Whether delivering individual counselling, facilitating group education sessions, or contributing to team discussions, you communicate complex nutritional information in a clear, supportive, and accessible manner.
You are committed to continuous quality improvement and professional growth, actively incorporating current research and best practice principles into your clinical work.
Highly organised and technologically proficient, you possess solid computer skills, including experience with CBORD and other clinical information systems, enabling you to support effective service delivery and accurate documentation.
With a current NSW Driver Licence and a dedication to improving outcomes for older adults, you bring professionalism, initiative, and a genuine commitment to excellence in dietetic practice.
