About the Role:
In this energetic position, you'll provide comprehensive healthcare support to participants in line with NDIS guidelines.
You'll oversee clinical care, medication compliance, and risk mitigation while leading a team committed to delivering high-quality, participant-centered care.
Key responsibilities include conducting health assessments, developing care plans, delivering training, and collaborating with internal and external stakeholders to achieve exceptional health outcomes.
Key Responsibilities:
Conduct detailed health assessments and develop individualised care plans.
Provide leadership to support workers, ensuring best practices in clinical care.
Deliver health-related education and training to participants and caregivers
Develop, implement, and regularly review individual care plans in collaboration with participants, their families, and Management.




Monitor participant progress and compliance with NDIS standards.
Respond to clinical incidents and manage risk mitigation strategies effectively.
Collaborate with multidisciplinary teams to enhance care quality.

About You:
Bachelor of Nursing - Registered Nurse with AHPRA registration.
Minimum of 3 years' experience in aged care or disability services.
Strong leadership, communication, and interpersonal skills.
Commitment to excellence in clinical care and continuous improvement.
Valid First Aid/CPR, WWC, NDIS Worker Screening, Police Check and Australian Driver's License.
Availability - Monday to Friday 9am to 5pm.
Available to work in both Salisbury and Prospect Offices.
