Why this role matters The Salvation Army Australia's Homelessness Stream delivers high-quality, person-centred services to individuals and families at risk of or experiencing homelessness.
We provide accommodation, case management, outreach, financial assistance, and connections to specialist services – supporting people to rebuild independence and stability.
About the role Join Foster House and make a meaningful difference in the lives of individuals and families experiencing homelessness.
As a Case Manager , you will provide trauma-informed, person-centred support that empowers clients to achieve housing stability, strengthen social connections, and build pathways to independent living.
Reporting to the Team Leader, this is a full-time permanent role based in Surry Hills.
Salary is in accordance with SCHADS Crisis Level 2.
How you will make an impact Deliver case management, crisis intervention, and transitional support in line with the National Homelessness Stream Model of Care.
Develop, implement,



and regularly review tailored case and care plans with clients.
Support residents and outreach clients through ongoing case management and advocacy.
Conduct risk assessments and safety planning for outreach and offsite activities.
Maintain accurate client records, case notes, and reporting requirements.
Collaborate with multidisciplinary teams and external agencies to achieve positive client outcomes.
Contribute to a safe, inclusive, and culturally responsive service environment.
What you will bring Diploma or higher qualification in Community Services, Social Work, or a related field (or currently working towards a degree).
Experience in case management and supporting people experiencing homelessness, mental health challenges, AOD issues, or other complex needs.
Sound understanding of trauma-informed practice and client-centred approaches.
Strong stakeholder engagement, communication, and documentation skills.
Ability to work collaboratively and adapt to changing service needs.
NSW Working with Children Check.
Willingness to complete a Nationally Coordinated Criminal History Check.
Current Driver's Licence (desirable).
Flexibility to travel and work varied hours as required.
