About the Role: Nirayu Disability Services is currently seeking to hire a Community Worker who meets the required qualifications and can contribute positively to the organization's values and commitment to high-quality care.
Responsibilities: As a Community Worker, you will provide essential care and support to individuals within their homes and the community.
The successful candidate will be responsible for assisting with daily living activities, promoting independence, ensuring the well-being of clients and supporting their participation in community life.
You will also help maintain a safe, respectful and supportive environment across the following tasks:
Deliver individualised support in accordance with client care plans.
Assist clients with personal care and everyday living activities.
Encourage independence, community participation, and social inclusion.
Observe client wellbeing and promptly report significant changes.
Maintain accurate client records and service documentation.
Build respectful relationships with clients, families, and support professionals.
Comply with organisational policies, workplace health and safety requirements and applicable care standards.
Support the delivery of reliable, compassionate, and high-quality community services.
Contribute positively to the organisation's service objectives and operational requirements.
Perform other duties reasonably associated with the position.
Requirements: The ideal candidate will hold a relevant Diploma or higher in a related field with 1-2 years of work experience.
At least three (3) years of relevant experience may be accepted as a substitute for the formal qualification.
