Wellways is seeking an experienced and community-focused leader to oversee the delivery and integration of Carer Gateway services across Rockhampton and Gladstone.
Carer Gateway is an Australian Government-funded program providing free support to unpaid carers - people who care for a family member, partner or friend living with disability, a medical condition, mental illness, alcohol or other drug concerns, or age-related frailty.
This role offers the opportunity to lead a multidisciplinary team, strengthen regional partnerships and improve access to support for carers across diverse communities.
The opportunity: As Service Integration Lead, you will oversee a team of approximately six employees delivering services including: Counselling Coaching In-person peer support Community engagement and service awareness Connections and referrals to other appropriate supports You will lead the integration of Carer Gateway services across Rockhampton, Gladstone and surrounding communities, ensuring the service is visible, accessible and responsive to local needs.
An important part of the role will be building relationships with hospitals, health services, community organisations and local providers.
You will create referral pathways, identify opportunities for collaboration and increase awareness of the support available to unpaid carers.
The position has a particular focus on reaching people who may experience barriers to accessing traditional services, including young carers, LGBTQIA+ carers, carers from culturally and linguistically diverse backgrounds, and people living in regional or more isolated communities.
Regional travel will be required, with a pool vehicle available for work-related travel.
Key responsibilities Lead, coach and develop a multidisciplinary team of approximately six employees Oversee safe, inclusive and person-centred Carer Gateway services Monitor performance against contractual requirements and program KPIs Coordinate service delivery across the Rockhampton and Gladstone catchment Build productive partnerships with health,
community and government stakeholders Develop local referral networks and improve community awareness Strengthen engagement with underrepresented and geographically dispersed carers Support Wellways' Wonders in the Community locations and activities Manage an operational and program budget of approximately $50,000 Travel across the service area to support partnerships, visibility and service integration About you: This role will suit an experienced leader who is equally comfortable supporting a team, managing service performance and representing an organisation across a regional community.
You are likely to bring: Experience leading multidisciplinary community or human services teams Knowledge of carer, mental health, disability, family support or related services Strong coaching, performance management and staff-development skills Experience delivering services against contractual requirements and KPIs Demonstrated community engagement and stakeholder-management capability The ability to establish regional partnerships and sustainable referral pathways A commitment to culturally safe, inclusive and person-centred service delivery Confidence working autonomously across a broad regional catchment The capacity to undertake regular travel between communities What is offered SCHADS Award Level 6 remuneration+ super Not-for-profit salary packaging Meal and entertainment allowance of approximately $2,600 Pool vehicle for work-related travel The opportunity to lead and develop a multidisciplinary team A role with genuine scope to improve access to support for carers across regional communities If you are a capable operational leader who can build strong teams, trusted partnerships and accessible community services, we welcome your application.
