What you'll be doing
IMHAOD Declaration
Each person's unique journey of recovery will be supported by mental health drug and alcohol services in a way that fosters hope, purpose and resilience.
IMHAOD Statement of Intention
The intention of the MNCLHD Integrated Mental Health and Alcohol and Other Drugs Services (IMHAOD) is to provide recovery-oriented, trauma informed services that are guided by evidence-based practices and collaboration.
The Transition and Physical Health Coordinator is primarily responsible for two primary functions.
Within the wider multidisciplinary team to coordinate, negotiate, and manage discharge planning needs of consumers to facilitate a smooth transition from hospital to the Community Mental Health Service and coordinate 7-day post discharge follow up clinics
Coordinate physical and metabolic care pathways ensuring the physical health of consumers is monitored and interventions are put in place.
About You
You bring qualifications that allow you to practise as a Social Worker eligible for membership with the Australian Association of Social Workers,



or as an Occupational Therapist recognised for registration with the Occupational Therapy Board of Australia.
You may also hold registration as a Registered Nurse with AHPRA, supported by relevant post‐registration qualifications and at least three years of clinical experience in your specialty area, or possess credentials that enable appointment as a Counsellor or Welfare Officer.
Across these pathways, you demonstrate a strong professional foundation and the capacity to operate confidently within complex clinical environments.
You are recognised for your clinical leadership, consistently showing the ability to work collaboratively within a multidisciplinary framework to plan, deliver and coordinate secure, effective and person‐centred care.
Your advanced clinical expertise enables you to make independent, well‐reasoned decisions that support positive patient outcomes, and you apply this judgement with a high level of accountability and professionalism.
You maintain a thorough understanding of the NSW Mental Health Act *****) and its Amendments, the National Mental Health Standards *****), and contemporary best‐practice guidelines for mental health service delivery.
You are either currently endorsed, or actively working toward endorsement,



as an Accredited Person under the NSW Mental Health Act, demonstrating your commitment to safe, lawful and evidence‐based practice.
Your capability extends to strong digital proficiency, using a range of clinical and administrative software systems—including patient flow platforms, word processing tools, data management systems and spreadsheets—to support efficient, accurate and timely service delivery.
You also hold a current unrestricted NSW Driver Licence and are able to travel as required, ensuring you can meet the needs of diverse service settings and community‐based care.
