About the role
We are inviting applications from suitably qualified candidates to join our My Community Home Care (MCHC) team in the role of Health Care Worker.
As a Community Health Care Worker, you will provide person-centred care, assisting our consumers to remain living independently in their own homes by providing assistance with domestic and personal care, social support, respite, and transport.
Provide safe, person-centred care that promotes consumer independence and dignity.
Assist with personal care, nutrition, hydration, continence care, and medication support as authorised.
Maintain accurate documentation and communicate changes in consumer condition.
Work in accordance with relevant legislation, regulatory requirements, quality standards, and organisational policies in aged care and/or community settings.

Remuneration in line with the Health & Allied Services, Managers and Administrative Workers Enterprise Agreement ($30-$33/hr based on qualifications and experience)
About you
You are passionate about what you do and want to support our consumers to live life to their potential, you also hold the following:
Certificate III in Individual Support/ Aged Care or equivalent is mandatory.
Current Drivers License
A clear and solid commitment to the providing quality care to our Consumers/Residents




Strong communication and consumer service skills
Well-developed interpersonal skills
Ability to prioritise tasks and problem solve
Able to comprehend and follow written instructions and procedures
Motivated and able to work effectively in a team environment and independently
All appointments are made subject to the required security screening including National Criminal History, NDIS Worker Screening checks andan immunisation status appropriate to the role, including the annual Influenza vaccination.
This is an onsite position that requires travel throughout surrounding communities.
