Tomaree Community Hospital in Nelson Bay is seeking a Level 3 Social Worker to provide high-level psychosocial support across acute and subacute settings, including complex case management and discharge planning.
You'll work within the Lower Hunter Social Work Team, with monthly supervision and a focus on psychosocial assessment, family meetings, guardianship applications, and independent practice.
