About the chance
Footprints is seeking a dedicated Link Worker to join ourSocial Support Services team, delivering the Social Health Connection Servicein conjunction with the Brisbane North Primary Health Network (PHN) across theMoreton Bay North sub-region (Caboolture and Kilcoy). Operating under a socialprescribing model, this outreach role focuses on empowering adults (18+) to improve their health and wellbeing by reducing social isolation and loneliness,centering on "what matters to me."
This is a one-year maximum-term contract, with thepossibility of extension. The annual salary for this position is approximately$92,279 (under SCHADS Awards) including superannuation, plus salary packagingbenefits of up to $18,550.
About you
Ideallyyou will have a Certificate IV or Diploma in Community Services, Social Work,Human Services (or similar field), be close to graduating or you will bringexperience delivering community-based support services. You will also bring thefollowing traits:
Abilityto be respectful, calm, and encouraging towards clients and have patience andunderstanding of others' journey, including the ability to set boundaries andbe self-aware.
Abilityto show initiative, think critically and make judgments based on sound reasonand the information/evidence presented.
Demonstratedcapacity to encourage and support individuals to engage in meaningfulactivities.
Goodwritten and verbal communication skills, and computer literacy
You will need
Arelevant Certificate IV or Diploma in relevant field (eg. Community Services,Social Work, Human Services) or significant progress towards such aqualification and demonstrated knowledge/experience of the provision ofcommunity-based support services.
Demonstratedexperience and understanding of the provision of support services to people whohave diverse needs.
Currentclass C driver license & own roadworthy vehicle
NationalNDIS worker screening check of current Yellow Card
