Join Liverpool Hospital as a Clinical Nurse Specialist Grade 2 within the Demand Management Unit, where you will play a vital role in supporting patient flow, facilitating timely discharges and contributing to positive patient outcomes across one of NSW's largest and busiest hospitals.
Work across the Demand Management Unit, Transit Lounge and inpatient wards to facilitate discharges and identify potential discharges.
Coordinate and progress discharge planning to support safe, efficient patient flow, and timely transitions of care.
Escalate issues that may impact discharge outcomes, including radiology requirements, patient transport and other barriers to discharge.
Apply your recent acute care nursing experience to support patient-centred care in a dynamic clinical environment.
Contribute to a Monday to Sunday service, collaborating with multidisciplinary teams teams (e.g. pharmacy; radiology; allied health and medical officers) -
Hours: Monday – Friday ************), Saturday – Sunday ************)

You are a proactive and detail-oriented clinician with strong communication skills and sound clinical judgement. Able to work autonomously while building effective relationships with multidisciplinary teams, you are committed to supporting safe and efficient discharge planning and patient flow.
Demonstrate strong attention to detail,
active listening skills and effective communication with multidisciplinary teams, including pharmacy, radiology, allied health and medical officers.
Apply sound clinical judgement to identify potential discharges and support timely patient progression.
Work autonomously within your scope of practice while recognising the need for timely escalation of issues and risks.
Show initiative in coordinating and facilitating discharge and transfer processes to achieve positive patient outcomes.
Identify and contribute to quality improvement strategies that support patient flow and meet the evolving needs of the hospital.
Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.
Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA) and Relevant post-registration qualifications and at least 3 years' experience working in the clinical area of the employees specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation.
