Competitive salary and benefits
Competitive salary and benefits: We offer attractive remuneration packages and generous leave entitlements.
Salary range: $79,661 to $109,546 p.a. (HWF3)
4 to 6 weeks annual leave with 14%-17.5% loading
*****% employer contribution to superannuation

About your opportunity
Join the Rheumatic Heart Disease (RHD)



team as a Health Worker (Identified), supporting culturally safe cardiac care for Aboriginal and Torres Strait Islander clients. You'll work with the multidisciplinary team across in-reach and outreach services, supporting health promotion, prevention and early intervention. You'll help lcients and families navigate the health system, strengthen health literacy and engagement, and provide cultural advice to improve cardiac outcomes. You'll also build connections with clients and communities, supporting continuity of care and helping ensure services are responsive to the needs of Aborigial and Torres Strait Islander people.
Requirements
Applicants must identify as Aboriginal and/or Torrest Strait Islander
Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice
Possession of a Class C driver's license
This is a Vaccine Preventable Disease (VPD) risk role
Probation period may apply
