About the Role:
Juniper is seeking experienced
Registered Nurses
to join our
Home and Community Care team
supporting clients across the
Perth metro area
.
In this role, you will provide clinical care and nursing support to clients living independently in the community. You will conduct assessments, develop and review care plans, and work collaboratively with clients, families and healthcare professionals to ensure safe, high-quality care at home.
This is a rewarding prospect for a compassionate and motivated nurse who is passionate about supporting older Australians to maintain their independence and wellbeing in the community.
Juniper is currently offering a casual contract; the successful candidate must be available Wednesdays and 1 weekend day a week.
As this is a community-based role, travel between client homes is required. Applicants must hold a current driver's licence and have access to a reliable vehicle.
Key Responsibilities:
Conduct clinical assessments and respond to changes in client health and wellbeing
Develop,
implement and review person-centred care plans with clients and their families
Provide clinical care including medication management, wound care and health monitoring
Support clients to maintain independence and wellbeing in their own homes
Liaise with GPs, allied health professionals and other service providers to coordinate care
Provide clinical advice and support to community care staff
Maintain accurate and timely clinical documentation and reporting
Ensure compliance with organisational policies and Aged Care Quality Standards
Participate in quality improvement and safe clinical practice initiatives
Key Requirements:
Current AHPRA registration as a Registered Nurse
Experience in aged care, community care or a similar clinical setting
Strong clinical assessment and care coordination skills
Ability to work independently in a community-based roleExcellent communication, organisation and problem-solving skills
Current driver's licence and willingness to travel between client homes
Current National Police Clearance (or willingness to obtain)
