Calvary Care is seeking a Clinical Project Lead for the Dementia Model of Care. You will lead the implementation across hospital, residential aged care, and home care services, collaborating with consumers, families and clinicians to drive person-centred, evidence-informed dementia care.

You will partner with service leaders to assess readiness, address barriers and sustain improvements, while building capability through education and tools for frontline teams.
