About the Opportunity

Bayley House is seeking an experienced and passionate Disability Support Worker (DSW) to join our Day Programs team.

As a Disability Support Worker, you will provide person-centred support to adults with intellectual disability, supporting participants to develop skills, build independence, engage with their community and work towards their individual goals.

You will work collaboratively as part of the Day Programs team to deliver high-quality supports that promote choice, dignity, inclusion and meaningful participation.

About the Role

As a Disability Support Worker within Day Programs, you will provide individualised support to adults with intellectual disability to participate in a range of meaningful activities and programs.

You will support participants to develop and maintain daily living, social, communication and community participation skills while promoting independence, choice and inclusion.

The role involves developing positive and respectful relationships with participants, understanding their individual goals and support needs, and working collaboratively with colleagues, families and other relevant stakeholders to deliver high-quality, person-centred support.

Key Responsibilities

- Provide individualised, person-centred support to adults with intellectual disability in accordance with their individual support plans, goals and preferences.




- Support participants to develop independence, confidence and skills through meaningful activities and experiences.
- Develop and facilitate structured activities and programs that reflect participants' individual goals, interests, abilities and support needs.
- Support participants to develop and maintain daily living, social, communication and community participation skills.
- Encourage participant choice, decision-making, dignity and inclusion.
- Provide personal care and assistance with daily living activities where required.
- Support participants with complex and high-intensity support needs in accordance with relevant plans and training.
- Support participants to safely access and participate in community-based activities and opportunities.
- Monitor participant wellbeing and progress and communicate relevant information to members of the support team.
- Maintain accurate and timely participant records, progress notes, incident reports and other required documentation.
- Work collaboratively with participants, families, colleagues, allied health professionals and other relevant stakeholders.
- Identify and appropriately escalation changes, concerns, incidents or risks relating to participant wellbeing.
- Comply with Bayley House policies and procedures, safeguarding requirements, NDIS Practice Standards and workplace health and safety requirements.
- Participate in training, supervision, team meetings and professional development as required.

About You

You are a compassionate and capable disability support professional who is committed to providing high-quality, person-centred support.

You understand that great disability support is about more than providing assistance – it is about building relationships, recognising individual strengths and supporting people to exercise choice, develop skills and participate meaningfully in their community.

You are reliable, adaptable and comfortable working both independently and collaboratively within a team environment.

You Will Ideally Have

- Previous experience supporting adults with intellectual disability.
- Experience providing person-centred support to people with a range of support needs.
- Experience supporting people with complex and/or high-intensity support needs, or a willingness to undertake relevant training.




- Experience developing and facilitating activities that support individual participant goals and interests.
- A robust understanding of dignity, choice, independence and inclusion in disability support.
- Strong interpersonal and communication skills.
- Ability to build positive and professional relationships with participants, families, colleagues and other stakeholders.
- Strong organisational and time management skills.
- Ability to maintain accurate participant documentation and records.
- Confidence using computers and electronic client management systems.
- An understanding of the NDIS, NDIS Practice Standards and relevant disability sector requirements.

Prior to Commencing, All Employees Will Require:

- Current NDIS Worker Screening Check.
- Completion of the NDIS Worker Orientation Module.
- Current First Aid and CPR Certificate, or willingness to obtain.
- Current Victorian Driver's Licence.
- Applicants must have full and unrestricted working rights in Australia for the duration of their employment with Bayley House. Applicants are required to provide evidence of their working rights as part of the recruitment process.
