What you'll be doing
Deliver and coordinate evidence-based, person-centred diabetes care, education, and support to patients/clients of Far West Local Health District, in accordance with clinical standards, policies, and procedures.
- Provide clinical support and supervision to staff and students where appropriate, to promote a culture of learning, capability development, and high-quality care delivery
- Maintain and develop professional knowledge and skills in diabetes care through ongoing education and reflective practice, to ensure contemporary and safe clinical practice
- Collaborate and communicate effectively with multidisciplinary healthcare teams to ensure continuity, integration, and quality of care across the patient journey.
