Registered Nurse - Drug Health Assertive Community Management Team

Where you'll be working
Join the Drug Health Assertive Community Management Team at Mt Druitt Centre for Addiction Medicine in this exciting district-wide opportunity. You will work alongside a dedicated multidisciplinary team supporting people experiencing complex health and substance use challenges in the community.

Be part of a specialised Drug Health service supporting vulnerable and diverse communities

Work collaboratively with nurses, clinicians and other health professionals

Contribute to a culture of respect, open communication and shared problem solving

Support people and their carers through compassionate, culturally appropriate care

Help drive continuous improvement and evidence-based practice across the service

What you'll be doing
As a Registered Nurse, you will provide safe, person-centred and evidence-informed nursing care while delivering assertive community-based Drug & Alcohol case management.

Assess, plan, implement and evaluate individualised nursing care

Provide comprehensive case management to people living in the community

Build therapeutic relationships with patients, carers and the multidisciplinary team

Maintain accurate clinical documentation, care plans, progress notes and referrals

Support the education and development of nursing staff and contribute to quality improvement

Current Nursing Registration (RN)



with the Nursing and Midwifery Board of Australia (AHPRA)

Selection Criteria

Demonstrated knowledge of drug and alcohol dependence issues and treatment interventions.

Immunisation Certificate from the College of Nursing or willingness to attain same

Current unrestricted NSW Drivers' license and willingnss to use WSLHD vehicle when necessary

Demonstrated effective interpersonal skills, excellent written and verbal communication skills within a multidisciplinary team

Evidence of participation in own professional development and commitment to maintaining competence and enhancement of skills, and willingness to apply and share professional expertise with colleagues

Evidence of computer literacy and understanding of relevant health related information systems

Demonstrated commitment to person centred care and NSW Health Core Values
