Eyre and Far North Local Health Network (EFNLHN) is seeking a part time ASO3 Case Coordinator or Enrolled Nurse to join the Elliston Community & Allied Health team as a Care Partner - Support at Home (Mid-West).
Based in the Eyre Peninsula region, you will develop and review individualised care plans, ensure service delivery aligns with Aged Care guidelines, and support participants to remain living independently at home, with supervision under the EN or ASO3 stream.
