About the role

The Diabetes Clinical Nurse Consultant provides self-management education to inpatients, outpatients and patients in the community living with diabetes. You may also be required to provide in-services to ward staff as required

Key responsibilities

Provide diabetes self-management education to patients on the wards to ensure a safe discharge

Support outpatient clinics seeing patients reviewed by the medical staff to reinforce safe blood glucose management

Provide home visits to assess patients in the community living with diabetes and their support persons.

Learn to support people with diabetes using technology such as CGM and insulin pumps

About you

You will be an RN with post graduate qualifications in diabetes education. You will be keen to work within a team but able to work autonomously as well.
