About the role: Western Health is seeking an experienced and compassionate clinician to join our Emergency Department Care Coordination (EDCC) Team on a 12-month fixed-term, full time basis . This is an exciting opportunity to work within a dynamic, multidisciplinary service that supports some of the most vulnerable members of our community by delivering innovative, person-centred care within the Emergency Department.
As an Emergency Department Care Coordinator , you will play a key role in improving patient outcomes by reducing avoidable hospital admissions, facilitating safe and timely discharges, and coordinating care for patients with complex medical, psychological and social needs.
Key responsibilities:
Working within a transdisciplinary team, you will provide comprehensive care coordination for patients presenting with complex needs, including those experiencing:
- Mental health challenges
- Substance use issues
- Homelessness or housing insecurity
- Disability
- Family violence
- Complex aged care needs
- Neurodiversity and behaviours of concern
- Significant psychosocial complexity
You will collaborate closely with emergency clinicians, inpatient teams,
community providers and external agencies to remove barriers to discharge, improve continuity of care and support patients to access appropriate services within the community.
About you:
You are a skilled clinician who thrives in a fast-paced environment and enjoys working collaboratively to solve complex problems. You have a genuine commitment to person-centred care and are passionate about improving outcomes for vulnerable populations.
You will bring:
- Qualifications in Nursing, Social Work, Occupational Therapy or Physiotherapy.
- Current registration with the relevant professional board or association.
- Demonstrated experience working with people experiencing complex health and psychosocial needs.
- Strong clinical assessment, care coordination and discharge planning skills.
- Experience working within multidisciplinary teams.
- Excellent communication, negotiation and stakeholder engagement skills.
- A trauma-informed, recovery-focused and culturally safe approach to care.
- The ability to think critically, prioritise effectively and work autonomously in a busy Emergency Department environment.
- Experience in Emergency, Acute Care, Mental Health, Community Health or Care Coordination will be highly regarded.
