Multicultural Care is a leading not-for-profit organisation providing culturally sensitive home and community-based support to older people, people with disabilities, and their carers. We are seeking an experienced and values-driven Care Partner who is responsible for the provisioning of high-quality case management, support planning, and care coordination services across all home care program types (including SAH and CHSP).

The position reports to the Manager Care Services and has an important role in providing support to the management team to ensure all care partnership activities is of primary focus to achieve set KRAs, KPIs and meets relevant compliance standards.

Key Responsibilities

- Deliver high-quality Home Care services that support clients to live independently and achieve their goals.
- Conduct client assessments and develop person-centred care plans tailored to individual needs and preferences.
- Provide case management, information, advice and referrals to support clients and their carers.
- Coordinate services and respond to changes in client needs, ensuring appropriate support is provided.
- Maintain accurate client records and complete documentation,



reporting and administrative tasks within required timeframes.
- Identify, manage and escalate risks, incidents and client concerns in line with organisational policies.
- Ensure compliance with relevant legislation, standards, policies and workplace health and safety requirements.
- Participate in professional development, team meetings and continuous improvement activities.

What We're Looking For

- Qualifications and/or experience in Health, Community Services, Aged Care, Nursing, or a related field. Clinical experience is highly regarded.
- Experience in case management, assessments, care planning, and budget management.
- Sound understanding of Consumer Directed Care and aged care reforms.
- Knowledge of aged care services and referral pathways.
- Excellent communication, customer service, and relationship-building skills.
- Robust computer literacy and experience using client management systems.
- Ability to work independently, manage competing priorities, and adapt to change.
- Understanding of the needs of older people, particularly those from CALD backgrounds.
- Current NSW Driver Licence and access to a reliable vehicle.
