Join our forward-thinking Virtual Care Team as a Community Nurse and play a vital role in supporting patients as they transition from hospital to home, promoting independence, and helping prevent unnecessary hospital admissions.
About The Role
As a Community Nurse within our innovative Virtual Care Team, you’ll combine your clinical expertise with virtual care technology to provide responsive, evidence-based and patient-centred care within the community.
You’ll work closely with patients, families and a multidisciplinary team to:
- Assess patient needs and develop individualised care plans.
- Support safe and timely transitions from hospital to home.
- Identify opportunities for early intervention and preventative care.
- Help reduce avoidable hospital presentations and admissions.
- Promote recovery, independence, health and wellbeing.
- Provide continuity of care through collaboration and effective communication.
- Use virtual care technology to enhance access, responsiveness and patient outcomes.

PLEASE NOTE: While this role utilises virtual care technology, it is not a remote or work-from-home position. The successful applicant will be site-based, reporting to a designated location within the Central Coast local health district. You will work collaboratively with patients, families, and a multidisciplinary team to assess needs, develop care plans, and implement strategies that promote recovery, independence, and overall wellbeing. With a focus on health promotion, early intervention, and continuity of care, your contribution will make a meaningful difference to the lives of those you support.

About You
- You hold a current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA), ensuring you meet the required professional standards.
- You have a passion for providing patient-centred care, with a strong commitment to improving health outcomes through innovative virtual care solutions.
- You are a skilled communicator, able to build strong relationships with patients, families, and healthcare providers to ensure safe, timely, and effective discharge planning.
- You hold a current Australian Drivers Licence with the ability to travel across the Central Coast
- You are highly organised and adaptable, with the ability to effectively use virtual technologies and evidence-based practices to support patients and manage complex care in a virtual environment
