Heal at Home is growing, and we are looking for an experienced and passionate Clinical Lead to join our team.

This is an exciting opportunity for a nurse who enjoys clinical leadership, supporting other nurses, building solid relationships with clients and referrers, and helping shape the delivery of high-quality community nursing services.

About the role

As Clinical Lead, you will provide clinical leadership and support to our community nursing team while continuing to maintain a strong clinical focus.

You will be responsible for:

- Providing clinical guidance, support and mentorship to community RNs and ENs
- Supporting nurses with complex clients and clinical decision-making
- Completing comprehensive nursing assessments and initial client visits where required
- Reviewing clinical care plans, documentation and clinical outcomes
- Supporting wound care, chronic disease management and complex clinical needs
- Providing clinical escalation and advice to the nursing team
- Supporting clinical governance, audits and quality improvement
- Identifying gaps in care and working collaboratively with clients, families and support providers
- Building strong relationships with referrers, support coordinators and other healthcare professionals
- Supporting the onboarding, development and competency of clinical staff
- Contributing to the ongoing development of our clinical systems, processes and standards
- Participating in the clinical leadership team and helping drive the growth of our community nursing service

About you We are looking for an experienced Registered Nurse who is confident working autonomously in the community and enjoys supporting and developing others.





You will ideally have

- Current AHPRA Registered Nurse registration
- Significant nursing experience, preferably within community, disability, acute or complex care
- Previous clinical leadership or senior nursing experience
- Strong clinical assessment and clinical reasoning skills
- Experience with complex wound management and chronic disease
- Excellent communication and interpersonal skills
- Strong documentation and clinical governance skills
- The ability to work independently while also being an effective team player
- A genuine commitment to person-centred, clinically appropriate care
- A current Australian driver's licence and reliable vehicle

Experience with NDIS clients and complex disability care would be highly regarded. Why join Heal at Home?
