About the role An experienced Accredited Practising Dietitian (APD) role that pairs remote case conferencing with hands-on clinic work, 1-2 days per week. The hybrid model combines structured multidisciplinary case conferences with in-clinic dietetic consultations, offering a more structured, team-oriented approach to chronic disease management with clinical variety.

Key responsibilities

- Contribute to remote multidisciplinary case conferences

- Develop and guide nutrition interventions for chronic conditions

- Provide evidence-based dietary recommendations

- Deliver in-clinic consults supporting care plan implementation

- Maintain high-quality clinical documentation and reporting

- Review patient histories and contribute to nutrition-focused care plans

- Provide education and follow-up aligned with care outcomes

About you

- Accredited Practising Dietitian (APD),



registered with Dietitians Australia

- Confident working remotely and independently

- Strong communicator, comfortable collaborating virtually

- Person-centred, empathetic approach to care

- Experienced with telehealth/hybrid service delivery

- Solid documentation and time-management skills

- Interest or experience in chronic disease management
