About the role

Being Human Plan and Care is seeking dedicated, compassionate and experienced Disability Support Workers to join our growing team across Ellenbrook, Midland, Applecross and surrounding areas. This is a casual position offering flexible work opportunities for individuals who are passionate about supporting people with disability to live safely, independently and with dignity. The successful applicants will provide person-centred support to participants in Supported Independent Living (SIL) and community settings, assisting them to achieve their individual goals and maintain their independence.

Key responsibilities

- Providing personalised support with daily living activities, personal care, meal preparation and household tasks

- Supporting participants within Supported Independent Living (SIL) environments

- Providing support with complex care needs in accordance with individual support plans and clinical instructions

- Supporting participants with PEG/PED feeding, catheter care and seizure/epilepsy management

- Assisting participants to attend appointments, social activities and community events

- Supporting participants to develop and maintain independence and community participation

- Following Behaviour Support Plans, health care plans and other participant-specific support plans

- Monitoring and reporting changes in participants' health, wellbeing, behaviour or support needs

- Maintaining accurate and timely participant documentation and progress notes

- Communicating effectively with families, Support Coordinators,



allied health professionals and other members of the support team

About you

- Certificate III in Individual Support (Disability) or equivalent

- Previous experience working directly with people with disability

- Demonstrated experience with PEG/PED feeding

- Demonstrated experience with catheter care

- Demonstrated experience supporting participants with seizures and epilepsy

- Ability to follow individualised care plans, health plans and clinical instructions

- Strong communication and interpersonal skills

- Ability to build respectful and trusting relationships with participants

- Current NDIS Worker Screening Check

- Current Working with Children Check
