We are recruiting experienced Home Care Workers to support older people returning home following a hospital stay. Across the North Shore and Northern Beaches areas.
Working within a short-term, rehabilitation-focused aged care program, you will help clients recover safely, rebuild their independence and achieve their individual goals while remaining in their own homes.
This is a permanent full-time position working Monday to Friday from 9:00 am to 5:00 pm, totalling 76 hours per fortnight.
Key responsibilities
- Provide respectful and person-centred personal care
- Support exercise programs as directed by allied health professionals
- Assist with compression stockings and higher-care needs
- Transport clients to appointments
- Provide social support and meaningful engagement
- Assist with meal preparation
- Complete domestic assistance and other practical home support
- Monitor client wellbeing and report any changes or concerns
- Maintain accurate service documentation
About you You will be an experienced and dependable Home Care Worker who is comfortable supporting older people with rehabilitation and post-hospital care needs.
To Be Considered, You Will Require
- Certificate III or higher in Individual Support, Aged Care or a related qualification
- At least six months’ experience in home care, aged care or a similar position
- Experience supporting clients with higher-care or post-hospital needs will be highly regarded
- Current Australian driver’s licence
- Reliable vehicle with comprehensive insurance
- Solid communication and documentation skills
- Ability to meet all mandatory screening and vaccination requirements
