East Wimmera Health Service (EWHS) is a fully accredited small rural health service across five campuses encompassing the five communities of Birchip, Charlton, Donald, St Arnaud and Wycheproof, in North Western Victoria.
An exciting opportunity is now available for a Home Support Assessment Officer to join our Community Health team playing a key role in the service delivery for consumers in the Home Support Program. The Home Support Assessment Officer will be working within the multi-disciplinary team to ensure the delivery of consumer centered care and will be responsible for the oversight and monitoring of consumer care plans, assessments, and risk management and mitigation.
The Home Support Assessment Officer will work alongside the Home Support Administration Officer in the initiation and ongoing delivery of home support services within the Buloke Shire to eligible consumers
Candidates must be able to demonstrate an ability to work independently and as part of a multidisciplinary team,
have advanced organisational and time management skills with excellent communication skills and the ability to build rapport with a diverse range of consumers.
