About the Opportunity An exciting opportunity is available for a Mental Health Clozapine Clinical Nurse Specialist 2 to join the Hunter Valley Lower Mid North Coast Mental Health Service, based in Taree.

The HVLMNC Mental Health Service provides acute community mental health care across Taree, Forster, Gloucester, Bulahdelah and the Hunter Valley, with teams including Care Coordination, Intake and Acute Care.

Working as part of an acute outpatient mental health service, you will provide specialist care to adults and their families across the community. The service supports consumers transitioning from hospital to community through medication monitoring, psychoeducation, relapse prevention, psychiatry reviews and collaboration with a range of internal and external support services.

This is an opportunity to take on a specialist role in Clozapine management while contributing to contemporary, recovery-focused mental health care.

What You Will Be Doing In this specialist role, you will:

- Provide specialist nursing practice, complex clinical care, advanced assessment and clinical leadership within Community Mental Health Clozapine Clinics and services.

- Coordinate the Clozapine Clinic at the assigned service and oversee satellite clinic service provision.

- Coordinate the care of consumers participating in the GP Shared Care Program.

- Provide complex case management and care coordination for Clozapine consumers.

- Provide specialist consultation and clinical support across a diverse community mental health environment.

- Deliver education and training to consumers, families, carers and health professionals.

- Support medication monitoring, psychoeducation, relapse prevention and continuity of care.

- Work collaboratively with psychiatrists, GPs, mental health clinicians and external support services.

- Provide mentoring, coaching and supervision to Registered Nurses.

- Contribute to quality improvement, clinical governance and the delivery of safe, evidence-based mental health care.

How You Will Be Supported

You will be directly supported by experienced senior leaders and the Team Manager, including the HVLMNC Mental Health Service Manager.

You will work closely with the broader HVLMNC Mental Health team, providing opportunities to collaborate, share knowledge and contribute to high-quality, contemporary community mental health care.

You will also be encouraged to participate in planned in-services,



as well as relevant virtual and face-to-face education and training to support your ongoing professional development.
