Who we are looking for

Focused on Care's Support Coordination client numbers have grown rapidly since we opened our Kempsey office earlier this year. This means we're now looking for an experienced NDIS Support Coordinator looking to join our existing team.

You'll be experienced in the providing high-quality client centre support coordination.

You're passionate about supporting people with disabilities and want to make a difference to people's lives.

You must possess the following skills/attributes:

- Experience in a support coordination, or similar role (essential)
- Passionate about providing person-centred and active support
- Drive and passion not only to support our existing clients but to contribute to the growth of this part of the business
- Ability to identify and implement workarounds to challenging problems.
- Excellent written and verbal communication skills
- Exceptional attention to detail
- Strong focus on quality outcomes and building relationships
- Experience in achieving KPIs
- Positive teamwork and customer service skills
- All relevant work authorities, or the ability to quickly obtain prior to commencing
- Authority to work in Australia

The following skills/attributes will be viewed favourably:
- Degree in Allied Health or a Certificate 3 or 4 in Disability, Youth Work, Welfare, Community Services, Mental Health and any other related courses

About the role The new team member will work closely with the Director and the Business Development team to continue to grow our client caseload in Kempsey, Coffs Harbour, Port Macquarie and everywhere in between.



This is a multi-faceted role with the successful candidate responsible for:

- assisting existing and new clients to implement their NDIS Plans and coordinate their services and supports
- building and maintain relationships with services providers and disability associations and groups in the region
- promoting the support coordination services of Focused on Care to the market and growing the client base

While this is an office-based regular travel will be required throughout the region to meet with our clients and stakeholders when required.
