As The Nurse Navigator
Diabetes, You Will
- Provide advanced, autonomous diabetes nursing practice and specialist clinical expertise, coordinating care for patients with complex diabetes and associated health conditions.
- Work across professional and organisational boundaries to connect patients, families, carers and healthcare providers, reducing fragmentation and improving the patient journey.
- Empower patients through tailored education, health literacy and self-management support, helping them make informed decisions and achieve their individual health goals.
- Contribute to service improvement, research and innovation by using evidence, data and contemporary practice to strengthen diabetes care and develop more productive models of care.
About You We are looking for an experienced and highly motivated Registered Nurse who brings advanced diabetes knowledge, strong clinical judgement and a passion for delivering integrated, person-centred care. You will bring:
- Demonstrated advanced clinical knowledge and contemporary experience in diabetes nursing, including the management and coordination of care for patients with complex needs.
- Strong communication, relationship-building and collaboration skills, with the ability to develop effective partnerships across primary,
secondary and tertiary healthcare settings.
- A commitment to evidence-based practice, quality improvement and service innovation, with the ability to collect and analyse data and translate evidence into improved patient outcomes.
- Transformational leadership capability, adaptability and a genuine commitment to engaging and empowering patients, families, carers and healthcare teams.

Unit Profile Bayside Health Service encompasses five public health campuses being Redland Hospital at Cleveland,
Marie Rose Centre at Dunwich North Stradbroke Island, Wynnum Manly Community Health Centre “Gundu Pa”, Pandanus Ward at Casuarina Lodge, Wynnum Manly and the Redlands Satellite Health Centre “Talwalpin Milbul” at Redland Bay.
Bayside Health
Service provides services within the hospital and community setting such as Emergency Care, Primary Health and Minor Injury Care, Acute and subacute medicine, general surgery, maternity and paediatric, mental specialist outpatient and mental health care.
