**About the role
Join the Patient Flow Services team at the Royal Brisbane and Women's Hospital and play a key role in improving outcomes for patients identified as being at risk of clinical deterioration. This advanced practice nursing position focuses on coordinating care for patients of concern, ensuring early intervention, timely escalation and seamless communication across the healthcare journey.
Working across a complex tertiary healthcare environment, you will act as a central point of coordination for patients, families and multidisciplinary teams. The role combines advanced clinical assessment, care coordination, leadership and education to support evidence-based care, improve patient outcomes and drive service improvements that enhance patient safety and experience.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Provide advanced nursing assessment, consultation and leadership for patients identified as being at risk of clinical deterioration or requiring complex care coordination.
Coordinate patient-centred care across the healthcare journey by reducing barriers, improving system integration and facilitating timely access to appropriate health and social services.
Act as a central point of communication for patients, families and multidisciplinary teams to promote collaboration and continuity of care.
Support patient engagement,
health literacy and self-management by facilitating informed healthcare decisions and advance care planning discussions where appropriate.
Lead quality improvement, education and service development initiatives that strengthen patient safety, evidence-based practice and outcomes for vulnerable patient groups.
