About the role:

As our Occupational Therapist / Restorative Care Partner, you will play an important role in helping our clients maintain their independence, confidence and quality of life.

You’ll work closely with clients to understand what matters to them, assess their needs and develop practical therapy and rehabilitation plans that support their goals. No two clients are the same, so this role is about more than just completing assessments. You’ll have the opportunity to be creative, problem-solve and find solutions that make everyday life easier and more enjoyable for our clients.

You will manage your own caseload and work alongside clients, families,



carers and other health professionals to achieve positive outcomes.

What you’ll be doing:

Occupational Therapy

- Complete person-centred OT assessments focused on daily living, wellbeing, independence and participation in the community
- Develop and deliver individualised OT and rehabilitation programs
- Support clients to achieve meaningful personal goals and maintain their independence
- Provide education, advice and practical strategies to clients, families and carers
- Assess and recommend equipment, aids and home modifications
- Regularly review client progress and adjust plans as required
- Communicate client needs and goals across different services and care settings
- Prepare clear and accurate clinical documentation, reports and recommendations
- Provide professional advice and support on OT and rehabilitation matters
- Contribute to quality improvement and community development initiatives

Restorative Care Partner – Support at Home

- Coordinate and oversee clients participating in the Support at Home Restorative Care Pathway.
- Work with clients, families and the multidisciplinary team to identify meaningful and achievable goals.
- Develop, implement and review individualised restorative care plans.
- Coordinate referrals, services and communication between clients, allied health, nursing, Care Partners and other stakeholders.
- Monitor client progress, identify changing needs or risks and coordinate appropriate referrals or reassessments.




- Lead exit planning and support clients to transition to ongoing services where required.
- Promote wellness, reablement, independence and a “doing with” approach throughout the restorative care journey.

What you’ll bring:

- Tertiary qualifications and current AHPRA registration as an Occupational Therapist
- Minimum 2 years’ post-registration experience working as an Occupational Therapist.
- Experience prescribing aids, equipment and home modifications
- Strong communication and interpersonal skills
- Excellent problem-solving skills and a practical approach
- The ability to work independently while being a outstanding team player
- Strong organisation and time management skills
- Confidence using Microsoft Office and client management systems
- A genuine commitment to person-centred care
- A commitment to the vision, mission and values of Melville Cares
- A current Driver’s Licence and roadworthy vehicle
- A National Police Clearance that is less than 3 months old
