About the role
The Navigator provides expert, autonomous care navigation and coordination for parents experiencing moderate to severe perinatal and infant mental health concerns, facilitating timely access to appropriate services across the South Australian PIMH care continuum.
Key duties of the role
Provide expert triage, care navigation and referral support to connect clients with appropriate perinatal and infant mental health and community services.
Actively participate in practice level multidisciplinary team meetings.eg.
Community of Practice
Hold a contemporary professional practice portfolio containing evidence of qualifications, learning and practice experience that underpin and demonstrable application of knowledge and skills commensurate with the level and type of practice expected of the role.
Contribute towards the development of the project, attending meetings and undertaking presentations.
Provide leadership and direction, acts a role model, mentor, consultant, and resource person relating to PIMH.
Skills and Experience
Registered Nurse and/or Midwife with current registration with the Nursing and Midwifery Board of Australia, or an eligible Social Worker or Psychologist.
Minimum 3 years' post-registration experience in nursing, midwifery, social work or psychology.
Demonstrated experience and competence in perinatal and infant mental health practice.
Demonstrated ability to undertake biopsychosocial assessment, risk assessment, care planning and intervention.
Strong communication, relationship-building and multidisciplinary collaboration skills, including knowledge of Aboriginal communities and culturally responsive practice.
