The Limestone Coast Local Health Network invites applications for a Senior Occupational Therapist in Mount Gambier.
You will join the Geriatric Outreach Service, delivering specialised OT to patients with dementia and cognitive impairment preparing for residential aged care transitions.
Collaborating with acute services, RACFs, families and carers, you will assess, plan and intervene to support protected discharges.
Travel across the Limestone Coast is required.
