Lutheran Services Woodridge, QLD, AU, is seeking a Clinical Care Partner to provide clinical oversight and mentor Care Partners, helping older people live safely and independently in their own homes.
The role is based at the Woodridge site with travel to satellite sites as needed.
You will lead complex care planning, review care plans, manage risks, and ensure compliance with aged care standards.
AHPRA registration, Nursing/Allied Health background, and a valid driver's licence are essential.
