The role of the Case Worker is to provide quality care to all clients in the Coolabaroo Housing Service and Coolabaroo Services.

We are seeking high level, passionate, enthusiastic team member/s who has case management and co-ordination experience.
Job Summary
This role is to assist and empower individuals, couples and families through a variety of support options, providing sufficient access to resources and support to ensure that clients are enabled to progress towards meeting their case management goals. The focus is upon client centred approaches within identified assessment processes and developed case plans. To support the Coolabaroo Housing Service Leadership Team, Executive Team and the CEO.
Bachelor/Diploma of Social Work and/or career experience in similar roles.
Experience with working with Aboriginal families, individuals and childrenn.
Experience in development of tools and systems to support facilitation of complex services within community services.
Demonstrated experience in Social and Community Housing and/or Domestic and Family Violence community services and/or Trauma Services and/or Cultural Healing Services and or National Disability Insurance Scheme.
Demonstrated leadership/supervisory experience working effectively and professionally in a multi-disciplinary team, and meeting contractual requirements.
Excellent written and verbal communication skills; and advanced interpersonal skills.
Computer literate in the Microsoft suite of programs and experience in information and technology platforms e.g Specialist Homelessness Information Platform (SHIP) online homelessness services portal from the Department of Communities.
Ability to maintain high level, comprehensive documentation (including case notes), reporting and assist with the review of internal policy and procedures.




Current Police Clearances and Working with Children Check Clearance.
Australian Citizenship, Residency Status or Visa allowing unrestricted work rights.
Self-directed, highly motivated, reliable and particular to detail.
Workplace health and safety policy and procedures are adhered to at all times.
Understanding of the National Principles - Child Safe Organisations.
Able to work collaboratively in a Leadership Team/s.
General Responsibilities
The position's success is determined by the ability to support Aboriginal clients, who are homeless, to obtain and establish and maintain safe independent accommodation. Support the Leadership Team with the establishment and case management of families inclusive of children. The Case Manager is a highly accountable, culturally safe leader who plays a key role in ensuring service delivery, has relevant tools and frameworks in place to ensure positive client outcomes. The Case Manager involves taking the lead in case management, linking clients to essential services and working collaboratively with the Leadership Teams.
KEY RESPONSIBILITIES
CLIENT SERVICES
Influence and contribute to the development and implementation of best systems practice to ensure high standards of service delivery, contractual compliance, and continuous quality improvement.
Provide Case Management to Aboriginal Families, individuals, children including Intake, Assessment, Planning, Implementation, Monitoring and Exit. Case mangement tools to be utlised, are to be culturally safe and in line with Trauma Informed Practice and ChildrenSafe Organisations principles. All interactions with clients are to encourage empowerment, self determination and independence from service provision.
Make appropriate referrals to internal and external agencies on behalf of clients to ensure culturally appropriate, culturally safe and holistic response to the family's, individual and/or child's needs.Participate or lead internal and external Case Reviews to form an integrated client support strategy.
Maintain accurate and timely record keeping, case notes, data collection and report writing (where relevant). Report critical incidents,



or clients at higher risk to the Coolabaroo Housing Service Leadership Team, Coordinator of Coolabaoo Housing and CEO.
Encourage and obtain client feedback to contribute to ongoing quality improvement of MGA services.
Develop referral networks and build strong collaborative networks to facilitate positive client outcomes.
Actively participate in regular formal supervision with the teams, to contribute to supervision of support workers and students as required.
Participate in Professional Development (PD). Participate in Performance reviews on a regular basis with Exteranal Human Resources.
ORGANISATIONAL MAINTENANCE
Contribute positively to Moorditch Gurlongga Association Incorporated Ethic and Values Statement, Vision and Mission.
Attend internal meetings, team building and forums as required
Comply with MGA Policy and Procedures at all times.
Work collaboratively and cooperatively within the CHS and CS Leadership teams contributing to a positive workplace environment and approaching any grievance with the guidance of internal policy and procedure.
Support a work workplace that is free from discrimination, harassment, bullying and refrain from engagement in any activities that may be offensive, humiliating, uncomfortable for; or derogatory towards; other staff or the community.
LEADERSHIP
Provide support to the Leadership Teams in areas of case work, referals, resources and cultural support.
Research and maintain awareness or issues relating to the Aboriginal community and to provide current, accurate and accessible information, resources and referral options for the range of potential client needs.
Providing ongoing support, mentoring and where appropriate, initiate professional development opportunities to ensure skills are maintained for self and colleagues.
Be an accessible and positive role model for the "Teams" of the client services, to access support and advise on client related aspects of their role.
Develop and maintain valuable professional working relationships with all members of the MGA Leadership teams.
The MGA teams, all support the day to day operations of CNC, CHS, CCS and CS as required.
Qualifications and experience required with positions, also Working with Children Card, National Police Clearance and a valid driver's licence.
Positions are paid under the Social Community, Home Care and Disability Services Industry Award ****.
