We're seeking a compassionate and skilled Clinical Care Coordinator in Hervey Bay to manage a clinical caseload, lead risk assessments, and coordinate holistic, person-centred care for clients with severe mental health conditions across the CCQ region. Working closely with a multidisciplinary team, you'll independently support smooth transitions between services, drive quality improvement, and contribute to positive client outcomes through ethical, collaborative practice.
Responsibilities
Conducting clinical assessments and developing tailored treatment plans
Deliver evidence-based clinical and psychosocial interventions
Autonomously manage caseload and conducting risk assessment with management plans
Collect data and supporting monitoring and evaluation
Collaborate with teams and external stakeholders
Facilitate care planning and review processes
Monitor and escape mental health needs as appropriate

Requirements
Tertiary qualification in Psychology, Social work, Mental Health Nursing, or similar
Current Mental Health Accreditation with AHPRA Registration or eligibility for AASW Membership




Experience working with a trauma informed, recovery focused approach supporting people with severe mental health challenges
Experience working with diverse cultures, especially First Nations people
Strong teamwork and collaboration skills, problem-solving and organisational abilities
