Full Time: $97,000 - $102,000 + Super + Salary Sacrifice
Healthii People are proud to have partnered with Open Minds a leading NGO, who in collaboration with other Mental Health Services are leading the development and oversight of the Clinical Care Coordination Program across the Country to Coast (CCQ) Primary Health Network (PHN).
These services are designed to provide a comprehensive, integrated and holistic approach to mental health care for adults with moderate - complex mental health conditions.
As a Clinical Care Coordinator, you will work within a multi-disciplinary team that is
not KPI centred
but more focussed on providing high quality mental health care to the participants.
Work within a multidisciplinary team
Conduct intake, risk assessments and safety plans
Provide short to mid term interventions and therapy that are evidence based




Coordinate care and provide holistic/ person centred case management

About you:
Relevant Qualifications in Social Work, Psychology, or MH Nursing
Accreditation with a governing body (AASW or AHPRA)
Knowledge of trauma informed therapeutic frameworks
Experience supporting adults with moderate to complex mental health challenges
Down to earth, humble, team player
