Clinical Care Partner
and help older people live safely and independently in their own homes. As a Clinical Care Partner, you'll provide clinical oversight and mentor Care Partners to deliver high-quality, person-centered care.
Location:
Working primarily across Woodridge and Toowoomba with some travel to satellite sites with mileage reimbursed.

What You'll Do
In this role you will support multiple sites both face-to-face and online, this is not a remote role, it will be based at our Woodridge site. Travel to other sites will be arranged between successful candidate and service, using a corporate vehicle or your own vehicle (with mileage paid).
Provide clinical oversight and case management support
Mentor and guide Care Partners
Review care plans to align with client goals and needs
Support clients, families, and health professionals
Manage clinical risks and complex care needs
Ensure compliance with aged care standards
What You'll Bring
Tertiary qualification in Nursing, Allied Health or a related clinical field




AHPRA registration
Experience in aged care, community or complex care
Proficiency in conducting complex clinical assessments, care planning, implementation, evaluation, documentation and budget management in line with the Aged Care Act
Strong understanding of risk management practices, including clinical risk assessment and mitigation
Strong communication and mentoring skills
A full
Australian driver's licence and a roadworthy, reliable vehicle
