Title: Nurse Practitioner

Unit/Team: Clinical and Nursing Services - Home & Community Based Nursing Team

Location: East Gippsland and Welllington Shires

Position type: Permanent EFT (negotiable)

Salary: $149,267 - $152,092 per annum (pro-rata), plus superannuation

The Position

- provide autonomous and collaborative clinical care while contributing to improved access to comprehensive assessment, care and support for older people and people experiencing changes in cognition, memory, communication, attention, thinking and problem-solving.
- Provide advanced nursing knowledge and skills in the assessment and management of clients with cognitive impairment and associated symptoms within the defined scope of practice and model of care.
- Establish and maintain strong collaborative working relationships with local GPs and key service providers through clear, concise, open and transparent communication.
- Facilitate interdisciplinary case meetings and care plan development, implementation and reviews as required.
- Facilitate an integrated service response that incorporates access to specialist care according to clinical priority, context and complexity and best practice care.
- Demonstrate comprehensive knowledge and understanding of, and practice in accordance with, professional standards, codes of conduct and relevant legislation and regulations as determined by the Nursing & Midwifery Board of Australia, Australian Nursing & Midwifery Council and other professional organisations relevant to the advanced practice role.

Essential Criteria

• Demonstrated experience of 5 years or more working in dementia or specialist aged care in a senior nursing role

• AHPRA recognised Registered Nurse qualification

• Endorsement as Nurse Practitioner

• Relevant experience in speciality area (or comparable role)
